This is to advise staff that when requesting information (e.g., DCFS 1726, Request for School Report; JV-450, Order for Prisoner's Appearance at Hearing Affecting Prisoner's Parental Rights; or DCFS 562, Health Care Visit Log) from a client or outside agency/individual, a self-addressed, stamped envelope should be provided as a courtesy to facilitate a timely response and receipt of the requested information.
