This is to provide Dependency Investigators (DIs) and PP CSWs with a hard copy of the Statewide Registry form for Probate non-related legal guardians.  Information on this registry was provided via FYI #00-56.

The Probate non-related must pay a registration fee of $385.00, payable at the time of the initial filing and at the re-register every three years thereafter by completing and submitting the SRCG-1 form.  The legal guardian may submit registration information via the Internet.  The web site address is http://caag.state.ca.us/conservator.  If the Probate non-related legal guardian does not have access to the Internet, the CSW should provide him/her with a copy of the attached registration form.

Reminder:  When a DI is completing a home study for a Probate legal guardian, inform him/her of their responsibility to contact the Statewide Registry, the penalties for non-compliance and the cost of the registration/re-registration.  PP CSWs should inform any Probate non-related guardians of the requirement and explain the fee for registration and re-registration.
