MEDICAID INFORMATION
Social Security has set up a centralized location where
individuals in receipt of Medicaid can call to request a replacement card or report
a change of address. They can also go online at the address below to request a
replacement card. At this time they are not yet set up for the change of address
online.

you have received benefits in the last 12 months
